Other insurances, such as Cigna, Aetna, GHI- patients may pay at the time of visit and a receipt will be provided to you so that you can be reimbursed by your insurance company.

Co-pays are due at the time of service. This is a requirement of your insurance company.
If you will owe a deductible or payment after the time of service, your insurance company will probably send you a statement indicating the amount you owe. Please send in your payment and please don't make me bill you. Or you can have me keep your credit card on file and I will charge it when I receive the info from your insurance company. If I bill you, my preference is to send bills by email.
Credit Card Authorization Form- you can choose a one time payment or to keep your numbers on file. Fax, email or mail it in.
